Denominator: Number of transitions of care during the EHR reporting period for which the provider was the receiving party of the transition.
Transition of Care: The movement of a patient from one setting of care (hospital, ambulatory primary care practice, ambulatory specialty care practice, long-term care, home health, rehabilitation facility) to another.
Medication Reconciliation: The process of identifying the most accurate list of all medications that the patient is taking, including name, dosage, frequency, and route, by comparing the medical record to an external list of medications obtained from a patient, hospital, or other provider.
How to Meet This Measure in Elation
This measure only applies to the first encounter with your patients after they have been seen by another health provider outside of your practice. For your visit to count as a “transition of care from other setting” and be in the numerator, there are 2 steps you need to perform.1. Select “From Other Setting or Dr.” in the Meaningful Use Requirements section of the bottom of the visit note to be counted in the denominator of this measure.
2. Then, to have the visit be counted in the numerator, either manually check off that “meds reconciliation was performed."
