This article will provide details on how to view a patient's medication history and take actions within the medication history panel.

A patient’s medication history can be viewed by clicking “Meds Hx” on the menu bar at the top of the patient chart and then clicking “Medication History”.
The complete Medication History box will then appear.
The Medication history can also be accessed through the clinical profile.
1.) Scroll down the Clinical profile until you come to the sections called “Permanent Rx Meds”, “Permanent OTC Meds” and “Scripts Since last 6 months”. A summary of the medications can be viewed here.
2.) To view a more detailed view of the individual medication click on the medication you wish to view.
3.) To view the full detailed medication history, click “View All Meds”. This brings you to the same view as described above by clicking on Meds Hx on the main menu tab at the top of the clinical chart.
Medications can be merged with other medications in the patient's list by completing the following steps.
1.)From within the clinical profile, click on the “Actions” button to the right of the medication you wish to merge.
2.)Scroll down the list of Actions and click “Merge with other Med”
Please Note: When you merge the two medications, Elation takes the name, dosage, and signature of the most recently documented scripts for the medication, and we re-purpose this as the name of the entire medication thread. Click here to see an example of this.
You can also access the Actions list by clicking “Meds Hx”.
3.) A box will appear for you to “Choose Med(s) to Merge with”. Simply select the appropriate med(s) and click “Merge with Selected Med(s)”
4.) The medication history box will appear with a yellow note “Meds merge complete” indicating the action has been completed.
To unmerge medications complete the following actions:
1.) Click Actions on the right-hand side of the medication you wish to unmerge(either by navigating to the medications list under the clinical profile section or by navigating to the meds Hx, as described above).
2.) Click “Move to other Med (un-merge)”
3.) In the box that appears you can select to “Set As Its Own Med” or merge with another medication.
4.) Click “Move order into Selected Med”
To delete a medication complete the following actions:
1.) Click Actions on the right-hand side of the medication you wish to unmerge (either by navigating to the medications list under the clinical profile section or by navigating to the meds Hx, as described above).
2.) Scroll down the menu and select “Remove Medication”
3.) Click “Yes, Delete” to complete the action
