[CQM] NQF 0028 Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention

Measure Description

Note: There is no % requirement for CQMs. However, it is best to report as much as you can for CQMs.

Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months and who received cessation counseling intervention if identified as a tobacco user.

Denominator:  Patients 18 years and older.
 
Numerator:  Patients who were screened for tobacco use at least once within 24 months AND who received tobacco cessation counseling intervention if identified as a tobacco user.

Exception: Documentation of medical reason(s) for not screening for tobacco use (eg, limited life expectancy, other medical reason).

How to Track This Measure in Elation

For the denominator to track correctly on Elation:
  • Make sure to record one of the following CPT codes in the visit notes: 96150, 96152, 97003, 97004, 99201, 99202, 99203, 99204, 99205, 99212, 99213, 99214, 99215, 99411, 99412, 99420, 99429, 99385, 99386, 99387, 99395, 99396, 99397, 99401, 99402, 99403, 99404, 90801, 90802, 90804, 90805, 90806, 90807, 90808, 90809, 90810, 90811, 90812, 90813, 90814, 90815, 90845, 90862, 92521, 92521, 92522, 92523, 92524, 92549, 92557, 92625, 96151
  • Make sure that the correct Primary Care Provider is listed for the patient, if your practice has more than one provider.

For the numerator to track correctly on Elation:

  • Click Address on "Tobacco Use Screening" MU Clinical Reminder OR fill out the patient's Smoking Status from the patient's clinical profile.
