GoalUse a CEHRT (Certified Electronic Health Record Technology) to generate a summary of care and electronically submit such summary to a receiving provider for more than 10% of transitions of care and referrals.
- Any provider who transfers a patient to another setting or refers a patient to another provider less than 100 times during the EHR reporting period is excluded from all three measures.
Numerator: The number of transitions of care and referrals in the denominator where a summary of care record was created using CEHRT and exchanged electronically.
Denominator: Number of transitions of care and referrals during the EHR reporting period for which the provider was the transferring or referring provider. Elation tracks this each time you select "To Other Setting or Dr." at the bottom of the visit note.

Please note that if you send a referral while the visit note is open, Elation will automatically check "To Other Setting or Dr."
Step 2: Attach the visit note to a Letter or Referral (Numerator)
Send a Letter to Provider or Referral and attach your visit note that was marked with "To Other Setting or Dr."
Add the recipient and their contact information in the To field.
Please include either a fax, Email, DIRECT address. If sending to another Elation provider, no contact information is necessary as the recipient will receive the data directly in their Elation account).
The Letter or Referral will not count if you print it; the letter must be sent from Elation to the receiving provider in one of the following formats:
- Existing ElationEMR/Provider Passport users
Elation will automatically count letters with the visit note attached towards the numerator. Whenever a Letter or Referral is sent to another provider, Elation creates an interactive electronic chart for the receiving provider to access online. By attaching the relevant visit note to the Letter to Provider, a summary of care is being transmitted electronically and made available to the receiving provider.
