PQRS: Qualified Clinical Data Registry-Based Submission

A QCDR (Qualified Clinical Data Registry) is different from a qualified registry in that it is not limited to measures within PQRS. A QCDR may submit measures from one or more of the following categories with a maximum of 30 non-PQRS measures allowed. This reporting method is for individual EPs only.

Reporting via QCDR is different from reporting via Qualified Registry

  • QCDR (Qualified Clinical Data Registry): A QCDR is a CMS-approved entity that collects medical and/or clinical data for the purpose of patient and disease tracking to foster improvement in the quality of care provided to patients. A QCDR will complete the collection and submission of PQRS quality measures data on behalf of EPs so that they may meet criteria for satisfactorily participating in 2015 PQRS. The data submitted to CMS via a QCDR covers quality measures across multiple payers and is not limited to Medicare.
  • Qualified Registry: A qualified registry is an entity that collects clinical data from an EP or PQRS group practice and submits it to CMS on behalf of the participants. 

How to Report using Qualified Clinical Data Registry

1. Select a Qualified Clinical Data Registry (QCDR) approved by CMS here
  • EPs participating via QCDR should work with their selected QCDR to determine how to participate
2. Report at least 9 measures covering 3 National Quality Strategy (NQS) domains for at least 50% of the EP’s applicable patients seen during the 2015 participation period.
  • Of the 9 measures, at least 2 must be outcome measures. If the QCDR does not possess 2 outcome measures, then the QCDR must possess at least 1 outcome measure and 1 of the following other type of measure: 1 resource use, OR patient experience of care, OR efficiency appropriate use, OR patient safety measure.
