Letters & Referrals Guide- Accessing patient information sent by an Elation EHR user

If you have received a fax or email from your colleague who uses Elation EHR, use the instructions in this article to gain secure online access to the patient information shared by your colleague.

  • Important Note: Access to view the shared patient records will expire 7 days after the day it was sent. If you need to reference these records for longer than 7 days, we recommend downloading the records to your computer.

I received a clinical fax

 

  1. You received a fax from your colleague and it looks like one of the following pages:

  1. Go to app.elationemr.com/efax and enter the Reference Code on the fax

  1. (First time only) Set up an account to view charts in Elation
    • Anyone in the recipient's practice can set up an account to view interactive charts in Elation.
    • The process is extremely quick: select whether you're a provider or staff member, enter your name, then set a password and security question. As an added convenience, Elation will help make sure your password meets our requirements as you type it.

I received an email invitation 

  1. You will receive an email from Elation notifying you that your colleague has shared information with you.

  1. Open the email and click the blue "View Shared Clinical Information" button.

  1. Select a Password and enter the 6 digit Provider Security Code. You must call the sending providers office to have them provide you with the 6 digit Provider Security Code.

  1. Once the two steps above are complete, click "I agree, Create My Account"

I already have an account



And I received a new fax and Reference Code

  1. Go to app.elationemr.com/efax and enter the Reference Code on the fax

  1. Select your name from the list if you already have an account

    • If you do not see your name in the list, click "My name is not listed here" to enter your name and view only the contents of this particular fax

  1. Enter your password and click "Go to Chart" to access all old and new records

  • If you forgot your password, click the "I forgot my password" button and then you will be prompted to answer the security question that you set up when your account was first created. After answering correctly, you will be able to reset your password.
  • If you do not remember the answer, click "I don't know the answer" and you will be able to view only the contents of the specific fax you are accessing (and not historical data)

And I received an email notification

Go to https://app.elationemr.com/login/ and click "Forgot password" to enter the email address associated with your log in to reset their password.



But I do not know how to find my account
 
  1. Search your email for the subject line "Welcome to the Elation Network!" to see if the connection was established through Email. If it was, Go to https://app.elationemr.com/login/ and click "Forgot password" to enter the email address associated with your log in to reset their password.
  2. If you did not connect through Email, go to https://app.elationemr.com/efax/locate-account/ and enter your last name and office fax number to try to locate your account
    1. Enter your password to log in if you have successfully located your account.
      1. If you forgot your password, click "I forgot my password" to answer your security question to locate your account.
        1. If you forgot your security question answer, click "I don't know the answer" to only view the most recent record sent to you (records expire after one week of being sent).
