The following information is provided for small practices (15 or fewer providers in a TIN) that report Traditional MIPS. Practices and providers that report using the APM Performance Pathways (APP- if you participate in an ACO), or submit and participate in MIPS Value Pathways (MVPs), those reporting options have different requirements. APP reporting offers full credit for Improvement Activities (no reporting needed), and MVPs have a smaller selection of Improvement Activities and Quality Measures to choose from.
Improvement Activities are one category of MIPS and worth 30% of the overall MIPS score if in a small practice and not reporting on the Promoting Interoperability category, and reporting Traditional MIPS . Total points for the Improvement Activities category caps at 40 and is dependent on a combination of scoring factors.
To earn full credit in this performance category as a small practice, you must generally submit one of the following combinations of activities:
- 2 medium-weighted activities (20 points each) OR
- 1 high-weighted activity (40 points)
Improvement activities have a minimum of a continuous 90-day performance period (during calendar year (CY) 2024) unless otherwise stated in the activity description. To submit Improvement Activities as an Eligible Clinician you can sign in and attest.
High-weighted activities receive 20 points and medium-weighted activities receive 10 points. There is a maximum of 40 points that can be earned from attesting to activities in this performance category. You’ll receive double points for each high- or medium-weighted activity you submit if you’re an individual clinician or group that holds a qualifying special status (small practices will receive double points).
With over 100 Improvement Activities to choose from, it can be overwhelming to decide which one is best suited for your practice. If you are in a small practice you only need to select one high weighted activity and attest to the activity in the QPP portal during MIPS reporting. Many of the activities capture best practices that you or your practice may already be doing.
