Elation Patient Passport Guide- Patient's Point of View

This article displays the patient's experience on the Elation Passport application for Patients, including the registration process and messaging functionalities. The video below also gives a run-through of the steps a patient takes to register for their Passport account after they have been sent an invitation their doctor's office and also describes the various Passport features.

Passport Registration Process

  1. Patients will receive an invitation email from the email domain "elationpassport.com" with the subject line [Doctor's Name] has a secure message for you
  2. Patient will open the email and click the "Click here to Register for Passport" button

  1. A new page will open up that prompts the patient to enter the 7 digit Passport Invitation Code and create a password. The 7 digit Passport Invitation Code will be texted to the cell phone number stored in their chart.
    • The patient must enter the 7 digit Passport Invitation Code, create password and enter it twice, agree to Elation's Terms of Service and specify if they are the patient, or a legal representative of the patient before they can complete registration.

  1. Once all fields are filled out, click the patient will click "Submit" to complete registration and then they will be brought into their Passport account.
 

Patient Communications Regarding Elation Passport

  1. Patient Passport Invitation Email- Sent to the patient when invited to Passport
  2. Patient Passport Registration Screen- The registration screen linked to the invitation email
  3. Signup Confirmation Email- Sent to the patient after they successfully register for Passport
  4. Patient Passport Welcome Letter- The welcome letter that appears in your patient's Passport when they first sign in
  5. New Message Notification Email- Sent to the patient each time new content is available in Passport
 

1. Patient Passport Invitation Email

When you invite a patient to sign up for Passport, the patient will receive the following email, asking them to register for their Passport account. There are also links to a 2-minute video that walks the patient through the registration process and a Frequently Asked Questions page.

2. Patient Passport Registration Screen

When your patient clicks "Click here to Register for Passport" in their invitation email (see email #1 above), they are automatically sent a text message containing the 7-digit invitation code required during the Passport registration process. Note that the invitation code will be sent via text message to the patient's mobile phone number on file in their chart and the invitation code is ONE-TIME use only for the registration process:

User Tip: The invitation code can also be found in the patient's chart by clicking on the "Passport" icon under the patient's profile picture and then clicking the "details" button in the Patient Passport box that opens up.

 

 

3. Signup Confirmation Email

When the patient successfully registers for Elation Passport, they will be sent an email confirming their successful registration. This email also provides them with instructions on how to access their Elation Passport account again in the future.

4. Patient Passport Welcome Letter

When the patient successfully registers for Elation Passport and logs in for the first time, they will see the following welcome letter in their inbox, which thanks them for signing up and reiterates that Passport is not to be used as a communication tool during emergencies.

  • User Tip: This Welcome Letter will only be used if you clicked the Passport button under the patient's profile picture to invite them to Elation Passport. If you invited the patient to Passport while writing your first letter to that patient, the letter that you wrote will serve as the welcome letter (and not this automatically generated letter).

5. New Message Notification Email

When a new message is sent to the patient from their Elation chart, the patient will receive a text or email notification depending on the Patient's Preferred Contact Method settings. Here is a copy of what the notification email looks like:

How patients log into their Patient Passport account

Patients can log into their patient passport account via the website app or by downloading the Elation Passport iOS application on their smartphone or iPad.

 

How patients receive message notifications

Each time that something new is sent to a patient's Passport account, the will be sent a simple email notification with instructions to sign into Passport to view the new content. These "new message" email notifications will be sent to the patient each time you send a new letter, write a new message on an existing letter letter, or sign off on a new visit note.

  • User Tip: Each time you sign a visit note, the visit summary is automatically shared with the patient in Passport.

How patients send a new message ​​​​​​

Patients are only able to send a new message if patient messaging is enabled. Click here to find out how to enable patient messaging.

  1. When you enable Patient Passport messaging, a large orange button appears at the bottom of a patient's “Timeline” page that says “New Message”:

  2. Clicking this button will open a blank form to compose a message to your practice. (The form clearly states at the top that Passport is only to be used for non-urgent communication).

  3. After the patient selects a message category, the message window will update and tell the user to whom their message will be routed at the practice.
    • If you have routed the category to a staff member, Passport will tell the patient that their message will go to "Staff at the office of...".
    • If you have routed the category to that patient’s provider, Passport will tell the patient that their message will go to their doctor.

  4. Patients have the opportunity to add attachments to new messages which is made apparent through the 'Add Attachments'  button.

  5. After the patient sends their message, a copy of the outbound message will appear at the top of their Timeline feed with an orange paper airplane icon:

How patients can self-schedule appointments on your practice's booking site

If you are using Elation's Booking Site feature, patients will see a direct link to your practice's Booking Site at the top of their Passport account to allow them to self-schedule appointments based on your availability. How patients use a single login to access multiple charts

The One Login feature allows patients to access multiple Passport accounts for different family members or Elation doctors using a single login to Elation Passport. This makes it easy for patient who a.) are responsible for the care of other patients on Elation (e.g. their children) or b.) have charts they wish to access from multiple Elation providers, to see and manage everything in one place. Anyone who has multiple logins to Elation Passport can consolidate the charts to a master Elation Passport login.
